You state your choice in the Donor Register. There are two ways of recording your choice.

1. You can use your DigiD on the website to enter your choice

2. Complete a paper form and send it by post

  • You can pick up this form from the municipality where you live. Or you can ask the Donor Register for one.
  • You complete a paper donor form.
  • Add the date and your signature to the form. Put the form in an envelope and post it back.

What happens if I don’t fill anything in?

If you don’t make a choice after receiving a letter: The register will state next to your name: ‘No objections to organ donation’.
In the event of your death, your family will then hear that you have no objection to donating organs and tissues. This means that your organs could be given to a patient after your death. The only way that it will be impossible for this to happen is if your family knows for certain that you did not want to be a donor and can show it.
