How do I record my choice?

You state your choice in the Donor Register. There are two ways of recording your choice.

How does it work?
Go to the website www.donorregister.nl, with or without your DigiD...
or with a paper form.
Record your choice on the website with your DigiD.
Go to www.donorregister.nl.
and click on "Record your choice".
Then click on "Log in with DigiD".
Log in with your DigiD credentials.
Your personal details will be filled in automatically.
Enter your choice for organ and tissue donation.
Have you ticked your selection?
Then click "confirm registration" to sign the form.
In a few weeks you will receive a letter...
containing confirmation of your choice.
Record your choice on the website without your DigiD.
Go to www.donorregister.nl and click on "Record your choice".
Then click "Complete form".
Enter your personal details and your choice on the form.
The submitted form will be sent to your home address.
Then you just need to add your signature...
and return it free of charge using the enclosed return envelope.
In a few weeks you will receive a letter containing confirmation of your choice.
Record your choice using a paper form.
Collect the paper form from the town hall...
or request one via the Donor Register.
Enter your personal details and your choice on the form.
Then sign the form...
put it in an envelope and post it to the freepost number indicated on the form.
In a few weeks you will receive a letter containing confirmation of your choice.

You can do that via www.donorregister.nl...
or by submitting another paper form.

2. Complete a paper form and send it by post

  • You can pick up this form from the municipality where you live. Or you can ask the Donor Register for one.
  • You complete a paper donor form.
  • Add the date and your signature to the form. Put the form in an envelope and post it back.

What happens if I don’t fill anything in?

The new Donor Act is into effect since 1 July 2020. If you don’t make a choice after receiving a letter: The register will state next to your name: ‘No objections to organ donation’.
In the event of your death, your family will then hear that you have no objection to donating organs and tissues. This means that your organs could be given to a patient after your death. The only way that it will be impossible for this to happen is if your family knows for certain that you did not want to be a donor and can show it.
