How do you record your choice?

You can record your choice by completing a donor form. That can be done via the website, with or without a DigiD. Alternatively, you can pick up a donor form from your municipality. You can also request a form from the Donorregister.

Recording your choice using your DigiD

  1. Log in with your DigiD.
  2. Go through the steps on our website to record your choice.
  3. The Donorregister will send confirmation of your choice by post within a few weeks.
  4. You will receive confirmation of your choice. You then have four weeks' time to check your choice and your details.
  5. After those four weeks, your choice is recorded. Doctors can then look up your choice in the Donorregister.

Recording your choice without a DigiD

  1. Go through the steps on our website to record your choice.
  2. Fill in the personal details correctly. You will receive your completed donor form by post within three weeks.
  3. Check your personal data and your choice. Add the date and your signature to the form. Send the form back to the Donorregister.
  4. The Donorregister will receive the signed form and confirmation of your registration by post within a few weeks.
  5. You have four weeks' time after receiving a confirmation of your registration to check your choice and your details.
  6. After those four weeks, your choice is recorded. Doctors can then look up your choice in the Donorregister.

What if I do not register?

Registration is not mandatory. If you do not record your own decision about donation, your partner or family will decide. That can be difficult if they do not know your choice. They may refuse to allow donation whereas you would have wanted to be a donor. That could mean organs and tissues are lost to people who are waiting for them.
